A 78-year-old woman, 4 days post-right hip hemiarthroplasty for a displaced femoral neck fracture, is admitted to your inpatient rehabilitation facility. On IRF day 2, nursing reports new onset quiet, withdrawn behavior, decreased oral intake, and difficulty with attention and orientation.
